Parent Checklist

Parents, these are the forms that we need to enroll all new and returning students each year. Please print a copy of each form that you need, and send or give them to the school. If it is a problem for you to print the forms, we can provide them for you. We need these before the first day of school. Feel free to call or email with any questions. Thanks very much.

• Enrollment Contract (parent)
• Student Enrollment Contract (for middle school students)
• Emergency Information Form
• Health Information - Parent
• Health Information B – Physician (The school requires a medical exam within 18 months of the start of school. Immunizations must be up to date every year. Contact the school for a waiver form if you wish to claim exemption from any immunizations.)
• Field Trip Release
• Photo Release
• Request for Transcripts (for new students only – parents give to sending school)
